the version number of UVG_AddVideo is the number number of UVG, I belive it isn't 3.0.0. please check it again.

So no special procedures to upgrade. Just go to Host > Module definitions and upload/install, right? Thank you!

yes. but if you use DNN 5, you should manually copy all dlls in the PA to your /bin folder after upgrade.
