When I load this module it removes all modules from the module drop down list. Deleting the Permission Manager modules allows the list to be populated again.

Please goto your default portal to set module permissions after installation.

I did go to my default portal, however are you looking for portal 0 or some other field in the database because my default portal is not on id 0.
